Embedded Smartsheet Forms
There are several benefits to embedding a Smartsheet form on an ASCO.org page rather than linking to a standalone Smartsheet form:
- Embedding a Smartsheet form into an existing page reduces friction for visitors, removing the need to click through to a separate page
- Embedded Smartsheet forms allow us to measure traffic to the form itself and provide insights into which marketing channels resulted in the highest traffic to the form.
- Linking to a separate ASCO.org “Thank You” page for the form upon submission (recommended) also provides additional information about the submission conversion rate, and which marketing channels drove the highest number of submissions.
Please use the following settings when creating your form to be embedded:
- In the area where you build the form, under Form Title settings, select “Do not show title and description”

- In the overall Settings, change the Theme to “Plain” and unselect “Show Smartsheet branding in footer”

- If using an ASCO.org thank you page (recommended) change the option for what should happen after form is submitted to “Send the user to link” – Your strategist or the ICM web team will be able to provide the correct link to use.

Once the form is created and the correct settings are applied, all that is needed is to provide the ICM web team with link to the form itself. From there, we will be able to embed the form on the page and test it to make sure it embeds cleanly and redirects to the correct thank you page. You will be able to manage the form and track submissions that come in as normal.
